Be sure to answer the **surcharge** questions (see below). Failure to do so will automatically add these charges to your monthly premiums for the 2010 calendar year.
PRINT A COPY OF YOUR CONFIRMATION PAGE TO TURN IN TO YOUR SCHOOL SECRETARY
Upcoming changes for all SHBP Members:
* Kaiser Permanente will no longer be offered
* There will be an increase in premiums of 10% for all plans
* Plan changes to all options (see Decision Guide)
**Spousal Surcharge will increase from $30 to $40**
**Tobacco Surcharge will increase from $40 to $60**
* Open Access Plans (OAP): CIGNA Open Access Plus and United Choice Plus (in place of the PPO). See page 6 of the Decision Guide for more information.
VERY IMPORTANT!
All changes to your current insurance coverage must be made by completing the appropriate forms provided below along with supporting documentation, if applicable. Requested changes will not be made without forms and documentation. No exceptions.
All forms must be turned in to your school secretary by Tuesday, November 10, 2009.
